Our social workers help patients and their families navigate everything rehabilitation touches beyond therapy itself, from insurance and benefits to planning for what happens after discharge. They're often the first people patients meet on the ward, and the ones who stay closest to the family throughout the rehab process.
Whether it’s help arranging home care, connecting with community resources, or simply someone to talk a hard decision through, the social worker is there. They also work closely with the medical and therapy teams to make sure the next transition, home or to another level of care, is as smooth as possible.
